Background:

  • Wegener's granulomatosis (WG) is a rare systemic vasculitis.
  • Prostatic involvement in WG is exceptionally uncommon.
  • WG can manifest with lower urinary tract symptoms.

Observation:

  • Two new cases of WG with prostatic involvement are presented, adding to five previously reported.
  • Patients exhibited obstructive urinary symptoms, proteinuria, leukocyturia, and hematuria.
  • Urinary findings resolved upon treatment of the underlying vasculitis.

Findings:

  • Granulomatous prostatitis is a rare cause of obstructive symptoms.
  • Antineutrophil cytoplasmic antibody (ANCA) titers correlated with clinical status.
  • ANCA titers predicted WG recurrence two months prior to clinical or Westergren sedimentation rate (WSR) changes.
