Area of Science:

  • Dermatology
  • Plastic Surgery

Background:

  • Pyoderma gangrenosum is a neutrophilic dermatosis characterized by painful ulcerative lesions.
  • It can be associated with systemic diseases or occur de novo.
  • Post-surgical pyoderma gangrenosum is an uncommon but recognized presentation.

Observation:

  • A 37-year-old woman developed pyoderma gangrenosum at surgical sites after reduction mammoplasty.
  • The clinical presentation mimicked infectious wound complications, leading to diagnostic delay.
  • Lesions were characterized by rapidly progressing, undermined ulcers.

Findings:

  • Intralesional triamcinolone acetonide injections were administered for treatment.
  • Complete resolution of the pyoderma gangrenosum lesions was achieved with this therapy.

Related Experiment Videos

  • Histopathological examination aided in differentiating from infectious etiologies.
  • Implications:

    • Early recognition of pyoderma gangrenosum features is crucial in post-surgical settings.
    • Distinguishing it from infection prevents ineffective treatments and patient distress.
    • This case highlights the importance of considering pyoderma gangrenosum in refractory surgical wound healing.
