Summary

A Salmonella enteritidis thyroid abscess in a diabetic patient resolved with antibiotics and drainage. The infection also caused temporary hyperthyroidism, which improved without specific treatment.

Background:

Observation:

  • A 70-year-old diabetic woman presented with a rapidly enlarging neck mass, fever, and symptoms suggestive of hyperthyroidism.
  • Ultrasound revealed a thyroid abscess, and fine-needle aspiration grew Salmonella enteritidis.
  • The patient had no gastrointestinal symptoms, indicating a non-enteric source of infection.

Findings:

  • Intravenous antibiotics (sulfamethoxazole/trimethoprim) and ultrasound-guided drainage led to abscess resolution within four weeks.
  • Transient hyperthyroidism, indicated by suppressed TSH and elevated fT4, resolved spontaneously with infection treatment.
  • Stool, sputum, and nasopharyngeal cultures identified the source of the hematogenous spread.

Implications:

  • This case highlights Salmonella enteritidis as a rare cause of thyroid abscess.
  • It demonstrates the importance of considering extraintestinal Salmonella infections in patients with fever and localized swelling.
