Summary

Physiologic measurement of diffusing capacity for carbon monoxide (DLCO) effectively predicts gas exchange impairment after pneumonectomy in foxhounds. Reduced DLCO significantly impacts oxygen transport during exercise, indicating diffusion limitation.

Area of Science:

  • Pulmonary Physiology
  • Respiratory Medicine
  • Exercise Physiology

Background:

  • Left pneumonectomy in foxhounds reduced diffusing capacity for carbon monoxide (DLCO) by 25-30%.
  • Despite theoretical predictions of no gas exchange impairment, experimental data suggest diffusion limitation develops during exercise post-pneumonectomy.

Purpose of the Study:

  • To determine if physiologic DLCO measurements can predict exercise-induced diffusion limitation to gas exchange after pneumonectomy.
  • To assess the relationship between DLCO and arterial oxygen saturation (SaO2) during exercise.

Main Methods:

  • DLCO was measured using the rebreathing technique and converted to diffusing capacity for oxygen (DLO2).
  • Arterial oxygen saturation (SaO2) was calculated based on DLO2 and measured physiological parameters (O2 consumption, alveolar PO2, hemoglobin, arterial pH) at various exercise intensities.

Related Experiment Videos

  • Calculated SaO2 was compared to observed SaO2.
  • Main Results:

    • Arterial oxygen saturation (SaO2) declined progressively with increasing exercise load after pneumonectomy.
    • The observed SaO2 decline during exercise was predictable from DLCO measurements at similar work loads (SaO2(Pr) = 22.73 + 0.77SaO2(Ob), r = 0.92).
    • The predictive model's slope being less than 1.0 suggests other factors, like ventilation-perfusion inhomogeneity, also contribute to arterial desaturation.

    Conclusions:

    • Physiologic DLCO measurement is a meaningful indicator of diffusion limitation to gas exchange.
    • In foxhounds, a moderate reduction in DLCO significantly impairs oxygen transport during exercise.
    • Post-pneumonectomy gas exchange abnormalities involve both diffusion limitation and other factors such as ventilation-perfusion inhomogeneity.
