Many smokers have undiagnosed chronic obstructive pulmonary disease (COPD). Early COPD diagnosis through spirometry in smokers over 40 is crucial for intervention and preventing disease progression.
Area of Science:
- Pulmonary Medicine
- Respiratory Health
- Diagnostic Screening
Background:
- Early identification of chronic obstructive pulmonary disease (COPD) is vital for preventing disease progression and respiratory failure.
- Smoking cessation programs are more effective when initiated early in the disease course.
Purpose of the Study:
- To determine the prevalence of undiagnosed chronic obstructive pulmonary disease (COPD) among current smokers.
- To assess the gap in COPD diagnosis within a high-risk population.
Main Methods:
- A screening procedure was conducted involving current smokers (≥8 cigarettes/day) identified through physician-certified sick leave exceeding two weeks.
- Lung function tests (spirometry) were performed on 3887 participants to diagnose COPD based on European Respiratory Society (ERS) guidelines.
Main Results:
- 17.3% (674/3887) of participants were diagnosed with COPD.
- Only 17.3% (103/674) of those with COPD had a prior physician diagnosis.
- Over 80% of COPD patients identified had no previous diagnosis, despite being on sick leave.
Conclusions:
- A significant proportion of current smokers have undiagnosed COPD.
- Increased awareness and routine spirometry for smokers over 40 are necessary for early COPD diagnosis.
- Spirometry is essential for accurate COPD diagnosis, not solely relying on reported symptoms.
