Background:

  • Constipation is a prevalent issue in individuals with intellectual disability (ID).
  • The underlying causes of constipation in this population are not well understood.
  • Colon transit time (CTT) measurement is a key diagnostic tool to differentiate types of constipation.

Purpose of the Study:

  • To compare total and segmental colon transit time (CTT) in patients with intellectual disability (ID) against healthy controls.
  • To investigate the specific patterns of CTT abnormalities in constipated versus non-constipated individuals with ID.

Main Methods:

  • Sixty patients with intellectual disability (ID) (IQ < 50) and 32 age-matched healthy controls participated.
  • Colon transit time (CTT) was assessed using radio-opaque markers and abdominal X-rays over seven days.
  • Data was analyzed using Bouchoucha's method to determine total and regional CTT.

Main Results:

  • Patients with ID exhibited significantly longer total CTT compared to controls (57.6 hours vs. 22.8 hours).
  • Constipated individuals with ID (MRC) showed prolonged CTT across all segments (right colon, left colon, rectosigmoid).
  • Non-constipated individuals with ID (MRNC) demonstrated prolonged rectosigmoidal CTT.

Conclusions:

  • Individuals with moderate to profound intellectual disability have significantly delayed total colon transit time.
  • Prolonged rectosigmoidal CTT in non-constipated ID patients suggests a defecation disorder.
  • The widespread CTT prolongation in constipated ID patients points to a diffuse colonic inertia problem.
