Summary

Tolbutamide treatment in burn hypermetabolism reduced serum glucose and improved nutritional status in rabbits. This suggests tolbutamide effectively mitigates the hypermetabolic state post-burn.

Area of Science:

  • Biochemistry
  • Metabolism
  • Endocrinology

Background:

  • Burn hypermetabolism is characterized by low serum insulin and tissue insulin resistance.
  • Understanding and mitigating hypermetabolism is crucial for burn patient recovery.

Purpose of the Study:

  • To evaluate the efficacy of tolbutamide in reducing burn hypermetabolism.
  • To investigate the impact of tolbutamide on key metabolic and nutritional markers post-burn.

Main Methods:

  • Adult male rabbits (n=18) with 30% TBSA full-thickness burns were used.
  • Tolbutamide (90mg/Kg/day) was administered orally to the treated group for 10 days.
  • Serum glucose, insulin, glucagon, oxygen expenditure, and nutritional indices were measured pre- and post-burn.

Main Results:

  • The tolbutamide-treated group showed significantly lower serum glucose levels (P<0.01).
  • Serum insulin levels and insulin/glucagon ratio were significantly higher in the treated group (P<0.01).
  • Improved cumulative nitrogen balance, oxygen expenditure, and serum albumin were observed in the treated group (P<0.05-0.001).

Conclusions:

  • Tolbutamide effectively reduces burn hypermetabolism in rabbits.
  • It likely acts by stimulating insulin secretion, inhibiting glucagon, and improving insulin sensitivity.
  • These effects promote glucose utilization, aiding recovery from burn-induced metabolic distress.
