Diffuse melanosis in metastatic malignant melanoma.

Summary

Diffuse melanosis in malignant melanoma patients is caused by pigment in skin macrophages and endothelial cells, not melanoma cells. This finding clarifies the cause of slate-blue skin discoloration in advanced melanoma.

Background:

Malignant melanoma can rarely present with diffuse cutaneous pigmentation, termed diffuse melanosis.
The precise mechanism and cellular origin of this pigmentation remain incompletely understood.

Observation:

Skin biopsies from a patient with diffuse melanosis secondary to malignant melanoma were analyzed.
Histopathological examination included light microscopy, immunohistochemistry, and electron microscopy.

Findings:

The slate-blue skin discoloration was attributed to melanin pigment deposition within dermal macrophages and endothelial cells.
Disseminated metastatic melanoma cells were not identified in the examined skin samples.

This study elucidates the cellular basis of diffuse melanosis in malignant melanoma.
It suggests that the observed pigmentation is a consequence of pigment phagocytosis and transport, rather than direct melanoma cell infiltration of the dermis.
