Summary

Semi-rigid and rigid endoscopes for transvaginal Natural Orifice Transluminal Endoscopic Surgery (NOTES) allow pelvic cavity access. However, these advanced scopes cannot visualize all anterior pelvic structures during gynecological procedures.

Background:

Natural Orifice Transluminal Endoscopic Surgery (NOTES) offers a less invasive surgical alternative.
Transvaginal NOTES procedures aim to access the abdominal cavity through the vagina.
Evaluating endoscopic instruments is crucial for advancing NOTES techniques.

Purpose of the Study:

To assess the utility of semi-rigid and rigid endoscopes in transvaginal NOTES procedures.
To determine the visualization capabilities of adjustable-angle endoscopes in the pelvic region.

Main Methods:

Four transvaginal gynecological NOTES procedures were performed.
A rigid endoscope (EndoCAMeleon) and a semi-rigid endoscope (EndoEYE LTF-VH) with adjustable viewing angles were utilized.
Procedures focused on evaluating transvaginal access, posterior peritoneum, ovaries, and tubal patency.

Main Results:

Transvaginal access into the abdominal cavity was demonstrated as feasible.
The posterior peritoneum, ovaries, and tubal patency were successfully evaluated.
Visualization of anterior pelvic structures remained a limitation with the evaluated endoscopes.

Conclusions:

Adjustable-angle rigid and semi-rigid endoscopes facilitate transvaginal NOTES access to the pelvic cavity.
These endoscopes do not provide complete visualization of all pelvic anatomical structures.
Further advancements in endoscopic technology are needed for comprehensive exploration in transvaginal NOTES surgery.
