Area of Science:

  • Minimally Invasive Surgery
  • Cardiovascular Surgery
  • Robotic Surgery

Background:

  • Robotic surgery offers potential for totally endoscopic coronary artery procedures.
  • The da Vinci system is being explored for coronary artery bypass grafting.

Purpose of the Study:

  • To describe the experience and outcomes of using the da Vinci robotic system for coronary artery bypass surgery.
  • To evaluate the feasibility and efficacy of robotic-assisted coronary revascularization.

Main Methods:

  • Study included 56 patients requiring single- or double-vessel revascularization.
  • Procedures performed off-pump on a beating heart: robotic mammary conduit harvesting with MIDCAB (minimally invasive direct coronary artery bypass) or TECAB (totally endoscopic coronary artery bypass) grafting.

  • Data collected on operating time, conversions, and patient outcomes.
  • Main Results:

    • 56 patients enrolled; 24 underwent MIDCAB, 23 underwent TECAB.
    • 9 patients (16%) required conversion to open surgery.
    • Mean operating time: 372 minutes for TECAB, 220 minutes for MIDCAB.
    • One-year follow-up complete; 92% overall graft permeability.
    • 3.5% mortality rate (1 hospital, 2 follow-up).

    Conclusions:

    • Robotic surgery is a feasible approach for coronary artery bypass grafting.
    • The da Vinci system demonstrates promising results in terms of graft patency and patient outcomes.
    • Further research may optimize this minimally invasive technique for broader application.
