Summary

Malignant phylloides tumors are rare in pregnancy. This case details the first malignant phylloides tumor diagnosed in the first trimester, enabling breast-conserving surgery and a successful pregnancy.

Background:

  • Malignant phylloides tumors (MPTs) are rare fibroepithelial neoplasms of the breast.
  • Pregnancy presents unique challenges for diagnosing and managing breast conditions, including rare malignancies.
  • Literature review revealed only six prior cases of phylloides tumors during pregnancy, none in the first trimester.

Observation:

  • A 27-year-old woman presented at 9 weeks gestation with a 24 mm right breast mass.
  • Core needle biopsy confirmed a malignant phylloides tumor.
  • The patient had a history of fibroadenoma excision from the same breast 7 years prior.

Findings:

  • The malignant phylloides tumor was surgically excised with clear margins.
  • Histopathology confirmed a 4-cm tumor with high mitotic activity.
