Bone Remodeling01:40

Bone Remodeling

Bone remodeling is a continuous and balanced process of bone resorption by osteoclasts and bone formation by osteoblasts. In adults, it helps maintain bone mass and calcium homeostasis. While mechanical stress can stimulate turnover as part of the normal maintenance and reparative process, several hormones also regulate bone remodeling.

Bone Remodeling and Repair

Osteoclasts are cells responsible for bone resorption and remodeling. They originate from hematopoietic progenitor cells present in the bone marrow. Numerous progenitor cells fuse to form multinucleated cells, each with 10-20 nuclei. A single osteoclast has a diameter of 150 to 200 µM. These cells have ruffled borders that break down the underlying bone tissue and release minerals such as calcium into the blood in bone resorption. Osteoclasts cling to bones with their ruffled edges during bone...

Area of Science:

  • Dentistry
  • Biomaterials
  • Bone physiology

Background:

  • Immediate loading of dental implants is a growing trend, driven by patient desire for faster functional restoration and fewer surgical procedures.
  • This approach is applied in cases of tooth extraction and replacement, as well as in edentulous sites.
  • Bone, a dynamic tissue, responds to surgical stimuli through modeling and remodeling processes.

Purpose of the Study:

  • To review the existing literature on bone remodeling in the context of dental implantology.
  • To identify parameters that influence bone healing and osseointegration.
  • To evaluate the impact of these parameters on the prognosis of immediate versus delayed implant loading.

Main Methods:

  • Literature review focusing on bone remodeling processes.
  • Analysis of factors modulating bone healing and osseointegration.
  • Evaluation of the bone phenotype's role in implant therapy prognosis.

Main Results:

  • Bone remodeling is a critical factor in osseointegration success.
  • Various parameters significantly modulate bone healing and influence immediate or delayed implant loading outcomes.
  • Individual bone characteristics (bone phenotype) are crucial for treatment planning.

Conclusions:

  • Understanding bone remodeling and individual bone phenotypes is essential for successful dental implant therapy.
  • The concept of "individualized implant therapy" is proposed to optimize treatment based on patient-specific bone characteristics.
  • This personalized approach aims to improve the prognosis for immediate and delayed dental implant loading.
