Summary

Routine repeat chest x-rays in trauma resuscitation are often unnecessary. This study found that repeat x-rays rarely yield clinically relevant findings, especially when the initial chest x-ray is normal.

Purpose of the Study:

  • To evaluate the clinical value of routine repeat chest x-rays in trauma patients.
  • To determine if repeat x-rays provide significant new information or alter patient management.

Main Methods:

  • Prospective collection of radiological imaging data for all patients in the TR over one year.
  • X-rays were counted and findings categorized as new injury, device verification, or injury progression.

Main Results:

  • Over 75% of 674 patients had two x-rays; 81% of repeat x-rays had no clinical impact.
  • Only 2.1% of repeat x-rays after a normal initial x-ray revealed new, non-clinically relevant injuries.
  • In cases with two abnormal x-rays, new diagnoses or deterioration were found in 22%.

Conclusions:

  • Routine repeat chest x-rays can be safely omitted in trauma patients with a normal initial chest x-ray.
  • Focusing resources on initial imaging may be more beneficial than routine repeat imaging.
