Background:

Conjunctival tumors present diagnostic challenges, requiring differentiation between primary lesions and secondary involvement from intraocular or scleral tumors.
Accurate diagnosis is crucial for appropriate management and prognosis.

Observation:

A patient presented with a conjunctival tumor, raising questions about its origin: a primary melanocytic conjunctival tumor versus scleral extension of an intraocular tumor with corneal infiltration.
Diagnostic uncertainty necessitated a careful evaluation to determine the tumor's extent and nature.

Findings:

A minimally invasive surgical approach was successfully employed for tumor resection.
The surgical procedure yielded excellent intraoperative and postoperative results.
No complications were observed during one year of combined ophthalmologist and oncologist surveillance.

Implications:

Minimally invasive surgery can be effective for managing complex conjunctival tumors with potential scleral or corneal involvement.
Multidisciplinary surveillance involving ophthalmology and oncology is vital for monitoring patients post-surgery.
This case underscores the importance of precise diagnosis in guiding surgical strategy for conjunctival neoplasms.
