Summary

Electrical perception threshold tests (EPT) can be abnormal in patients with voiding disorders, independent of bladder sensation. These sensory tests offer complementary diagnostic information for lower urinary tract dysfunction.

Purpose of the Study:

  • To compare different sensory tests used in evaluating lower urinary tract (LUT) sensation.
  • To investigate the relationship between bladder filling sensations and electrical perception threshold tests in patients with voiding disorders.

Main Methods:

  • Seventy patients with functional LUT disorders underwent cystometry, bladder electrical perception threshold (bEPT), urethral electrical perception threshold (uEPT), pudendal electrical perception threshold (pEPT), and pudendal somatosensory evoked potentials (pSSEP).
  • Correlations were analyzed between somatosensory tests (uEPT, pEPT, pSSEP latency) and viscerosensory tests (filling sensations, bEPT).

Main Results:

  • A significant portion of patients (71%) had normal bladder filling sensations but abnormal bEPT.
  • Abnormal filling sensations were observed in 29% of patients, often accompanied by increased bEPT.
  • No significant correlation was found between bladder filling sensations and bEPT, but pEPT correlated with uEPT and pSSEP.

Conclusions:

  • Electrical perception threshold (EPT) testing can reveal sensory disturbances independent of bladder filling sensation in voiding disorders.
  • Bladder filling sensation volumes and bEPT are not significantly correlated, indicating complementary diagnostic value.
  • Somatosensory tests of the pudendal nerve show correlation, suggesting limited added benefit from performing all tests individually; they cannot quantify filling sensation.
