Summary

Tacrolimus, a common immunosuppressant, can cause small intestinal angioedema in kidney transplant patients. This is the first reported case diagnosed using capsule endoscopy, highlighting a potential adverse effect of this drug.

Background:

Angiotensin converting enzyme inhibitors are known to cause small intestinal angioedema.
Tacrolimus is a widely used immunosuppressant in organ transplantation.
Small intestinal angioedema has not been previously associated with tacrolimus therapy in transplant recipients.

Observation:

A kidney transplant patient presented with severe diarrhea.
Abdominal computerized tomography and capsule endoscopy were utilized for diagnosis.
The diagnostic imaging revealed intestinal angioedema.

Findings:

This case represents the first documented instance of tacrolimus-induced small bowel angioedema.
Capsule endoscopy proved effective in diagnosing the condition.
The patient's symptoms were attributed to tacrolimus treatment.

Implications:

Clinicians should consider tacrolimus-induced intestinal angioedema in transplant patients with gastrointestinal symptoms.
Capsule endoscopy is a valuable tool for diagnosing small bowel angioedema.
This finding may necessitate a re-evaluation of tacrolimus safety profiles in certain patient populations.
