Background:

Ejaculatory/orgasmic disorders are common male sexual dysfunctions.
Includes premature ejaculation (PE), inhibited ejaculation, anejaculation, retrograde ejaculation, and anorgasmia.

Framework:

International consultation with over 200 multidisciplinary experts from 60 countries.
Recommendations developed over a 2-year period based on evidence-based literature and expert opinion.

Implementation:

Premature ejaculation management depends on etiology: pharmacotherapy (SSRIs, topical anesthetics) for lifelong PE, etiology-specific treatment for acquired PE.
Behavioral therapy indicated for psychogenic factors, often combined with pharmacotherapy.
Retrograde ejaculation managed by education, reassurance, pharmacotherapy, or surgery.
Delayed ejaculation, anejaculation, and anorgasmia require biogenic/psychogenic etiology assessment.
Age-related penile hypoesthesia managed with education, reassurance, and revised sexual techniques.

Implications:

Current state-of-the-art knowledge for managing male ejaculation/orgasmic disorders.
Highlights the need for further research into these conditions.
