Summary

Sonographically guided injections into the popliteus tendon sheath are accurate. Both longitudinal and transverse approaches show high success rates, though overflow into the knee joint can occur.

Background:

The popliteus tendon sheath is a crucial structure in the posterolateral knee.
Accurate injection into the popliteus tendon sheath is vital for targeted therapies.
Understanding optimal injection techniques is essential for clinical practice.

Purpose of the Study:

To describe two sonographically guided popliteus tendon sheath injection techniques.
To evaluate the accuracy of these techniques in a cadaveric model.

Main Methods:

24 sonographically guided popliteus tendon sheath injections were performed by one operator.
Injections utilized either a longitudinal or transverse approach relative to the tendon.
Latex was used as the injectate, and specimens were dissected to assess accuracy and overflow.

Main Results:

The longitudinal approach achieved 100% accuracy in placing latex within the sheath.
The transverse approach achieved 83% accuracy in placing latex within the sheath.
Both techniques demonstrated a risk of injectate overflow into the knee joint.

Conclusions:

Sonographic guidance enables accurate popliteus tendon sheath injections.
The longitudinal approach may offer slightly higher accuracy.
Injectate overflow into the knee joint is a potential complication with both techniques.
