Context:

  • Leukoaraiosis (LA) is a common finding in elderly individuals, often associated with cognitive decline.

Purpose:

  • To investigate the relationship between DTI metrics (average diffusion coefficient - DCavg, fraction anisotropy - FA) of LA lesions and LA severity.

  • Cognitive function (MMSE) showed significant relationships with DCavg and FA in normal-appearing white matter (NAWM) of LA patients, particularly in the anterior horn and centrum semiovale.
  • DTI revealed characteristic changes in DCavg and FA for LA lesions, indicating microstructural alterations.

  • DTI can detect white matter microstructural changes in LA that are not apparent on conventional MRI.
  • These DTI-derived microstructural changes are associated with cognitive impairment in LA patients.
