Background:

Rural South Africa's primary healthcare relies on remote clinics and health centres.
Health centres were established in 1994 to act as a crucial intermediary between community clinics and district hospitals.

Purpose of the Study:

To document the clinical issues at a new health centre in a deprived area.
To compare these issues with those at an adjacent community clinic and district hospital.

Main Methods:

A cross-sectional survey was conducted in the Eastern Cape Province, South Africa.
Data from 4,383 patient encounters (2001) at a community clinic, health centre, and district hospital were coded using the International Classification of Primary Care-2 (ICPC-2).

Main Results:

The health centre recorded the highest patient throughput, functioning effectively as a healthcare filter.
Respiratory issues were the most common diagnosis (23%), with cough as the most frequent symptom.
Nurses provided most care (80% at health centre, 97% at clinic), and 30% of children under five received immunizations at the health centre.

Conclusions:

The health centre demonstrated high patient throughput, confirming its role as an efficient healthcare filter.
The International Classification of Primary Care-2 (ICPC-2) proved suitable for monitoring patient encounters in similar African healthcare settings.
