Team and problem oriented approach in geriatrics: some practicalities

Those involved in rehabilitation of the older person must accept the challenge of multiple pathology, complex medical, psychological and social dysfunction and the need for reorientation of the person to new social roles. Health professionals with different specific expertise are needed to identify the problems and to work out solutions. Coordination of the efforts of such experts, and inclusion of the patient and family in planning therapy are essential. Two approaches are being developed at St. Peter's Centre to meet these requirements: the problem oriented record and team management. This article describes the practical application of these two concepts.
