Intraventricular haemorrhage (IVH) can be fatal. A novel treatment using rt-PA through an external drain successfully cleared brain bleeds, leading to good patient recovery.

- Complete clearance of the ventricular system from blood was achieved within days.
- The patient experienced a favorable neurological recovery post-treatment.
Implications:
- This case suggests rt-PA is a viable option for IVH clot lysis.
- Intraventricular thrombolysis may improve outcomes in patients with severe IVH.
- Further research into rt-PA for IVH is warranted to establish efficacy and safety.
