Summary

Endothelin-1 (ET-1) expression in prostate cancer (PCa) biopsies is a significant predictor of extracapsular stage (pT3a). This finding aids in managing localized PCa by identifying patients who may benefit from specific treatments.

Area of Science:

  • Oncology
  • Pathology
  • Biomarker Research

Context:

  • Prostate cancer (PCa) staging is crucial for treatment decisions.
  • Accurate prediction of extracapsular extension (pT3a) is vital for localized PCa management.
  • Identifying reliable prognostic factors in PCa biopsies is an ongoing area of research.

Purpose:

  • To evaluate endothelin-1 (ET-1) immunohistochemistry expression in prostate cancer biopsies as a prognostic factor for extracapsular stage (pT3a).
  • To compare ET-1 expression between pT2 and pT3a stages of prostate cancer.
  • To determine the independent predictive value of ET-1 expression for pT3a stage.

Summary:

  • Sixty-eight radical prostatectomies for localized PCa (pT2 and pT3a) were analyzed.
  • Higher initial PSA and biopsy Gleason score > or =7 were associated with pT3a stage.

  • Significantly higher ET-1 expression was observed in PCa biopsies and specimens from the pT3a group compared to the pT2 group.
  • Biopsy ET-1 expression was an independent risk factor for pT3a stage, with high predictive values.
  • Combined ET-1 expression and initial PSA improved predictive accuracy for pT3a stage.
  • Impact:

    • Endothelin-1 (ET-1) biopsy expression serves as an independent prognostic factor for extracapsular stage (pT3a) in prostate cancer.
    • This research may inform clinical decisions regarding active surveillance, curative treatment, and adjuvant therapy for localized PCa.
    • Further studies on ET-1 expression could refine risk stratification and personalize treatment strategies for prostate cancer patients.
