This case report details a rare instance of Salmonella infection causing aortoduodenal fistula, a serious complication of aortic pseudoaneurysm. Prompt surgical intervention led to an uncomplicated recovery, highlighting the importance of timely diagnosis and treatment for this condition.
Area of Science:
- Vascular Surgery
- Infectious Diseases
- Gastroenterology
Background:
- Primary aortoenteric fistula typically results from abdominal aortic aneurysm erosion into the bowel.
- Septic aortitis leading to pseudoaneurysm and subsequent fistula formation is an exceptionally rare etiology.
- Salmonella-induced aortitis presenting as an aortoduodenal fistula is exceedingly uncommon in medical literature.
Observation:
- A 51-year-old male presented with sepsis secondary to Salmonella bacteremia.
- Imaging revealed an infrarenal aortic pseudoaneurysm.
- Emergency laparotomy identified a fistula between the duodenum and a pseudoaneurysm of a severely infected aorta.
Findings:
- Surgical excision of the infected aortic segment and repair of the duodenal defect were performed.
- Aortic stumps were sutured, and an axillobifemoral bypass was established.
- The patient experienced an uncomplicated postoperative recovery.
Implications:
- This case underscores the potential for Salmonella aortitis to cause aortoduodenal fistulas.
- Highlights the critical role of prompt diagnosis and aggressive surgical management in improving patient outcomes.
- Contributes rare insights into the management of infectious aortoenteric fistulas.
