Background:

Laparoscopic Kugel patch placement via preperitoneal approach is preferred for less invasive hernioplasty.
Initial attempts using total extraperitoneal approach faced technical challenges.

Purpose of the Study:

To evaluate the feasibility, safety, and efficacy of a laparoscopic intraperitoneal onlay approach for Kugel patch hernioplasty.
To assess this method for treating inguinal hernias, including recurrent cases.

Main Methods:

Laparoscopic hernioplasty performed on 5 patients using the Composix Kugel Patch.
Mesh fixation involved titanium spiral tacks on the polypropylene side and polydioxanone sutures.
Monolateral hernias were treated, with one recurrent case.

Main Results:

No intraoperative complications or need for conversion to open surgery.
Mean surgical duration was 77 minutes.
One small postoperative seroma was the only complication; pain management was generally effective.

Conclusions:

The laparoscopic intraperitoneal onlay approach is a feasible, safe, and effective option for inguinal hernia treatment.
This method shows promise as a safer alternative to other laparoscopic hernioplasty techniques, especially for recurrent hernias.
