Background:

  • Cap-assisted endoscopic mucosal resection (EMR) is established for upper GI cancers.
  • Limited data exists on its use for colorectal lesions.

Purpose of the Study:

  • To assess the safety and efficacy of cap-assisted EMR.
  • To evaluate its use in treating colorectal sessile polyps and lateral spreading tumors.

Main Methods:

  • A retrospective review of 255 patients treated between 2000 and 2007.
  • Inclusion criteria: sessile polyps or lateral spreading tumors >= 20 mm.

Main Results:

  • 146 sessile polyps and 136 lateral spreading tumors were treated.
  • Overall complication rate was 8.6%, with 7% intraprocedural bleeding (controlled endoscopically). No perforations occurred.
  • Recurrence rate was 4% after a median follow-up of 12.1 months.

Conclusions:

  • Cap-assisted EMR is effective for colorectal sessile polyps and lateral spreading tumors.
  • Piecemeal resection necessitates close endoscopic surveillance for complete lesion removal.
  • The technique offers a safe and effective treatment option with manageable complication rates.
