Background:

  • Total hip replacement (THR) with metal-on-polyethylene (MoP) bearings is a common orthopedic procedure.
  • Wear debris from MoP implants can potentially lead to adverse tissue reactions.
  • Pseudotumoral lesions are a rare but serious complication following THR.

Observation:

  • Three cases of pseudotumoral lesions secondary to THR with MoP bearings were observed.
  • Lesions presented with two distinct macroscopic appearances: classic inflammatory granuloma and hematoma with gluteal vessel lesions.
  • Radiographic and CT-scan imaging were essential for diagnosis, extension assessment, and surrounding tissue analysis.

Findings:

  • Histological analysis confirmed granulomatous lesions related to wear debris in all cases.
