Background:

  • The Sundt clip graft is a specialized device for repairing critical tears or dehiscence in intracerebral vessels.

  • A retrospective review identified 17 patients treated with the Sundt clip graft between 1988 and 2008.
  • Repairs were performed on various vessels, including those affected by carotid aneurysm blisters, anterior choroidal artery (ACA), posterior communicating artery (PCA), and middle cerebral artery (MCA).

Findings:

  • Postoperative angiography in 16 patients showed successful vessel preservation with at least 80% patency in 14 cases.
  • Neurological outcomes varied, with 13 patients achieving a good Glasgow Outcome Score (GOS) of 5, while 3 had poor outcomes (GOS 3) and 1 died due to subarachnoid hemorrhage (SAH) complications.

Implications:

  • The Sundt clip graft is a viable option for neurosurgeons managing emergent vascular injuries during surgery.
  • Its use can be critical when conventional repair methods are not feasible, potentially improving patient outcomes in complex cases.
