Background:

  • Atherosclerosis is an early-life process influenced by multiple risk factors, including hypercholesterolemia.
  • Coronary heart disease (CHD) arises from complex interactions among various risk factors.

Purpose of the Study:

  • To assess lipid abnormalities in children identified with elevated total cholesterol during routine screening.
  • To determine the prevalence of specific lipoprotein phenotypes and low high-density lipoprotein cholesterol (HDL-C) in this pediatric cohort.
  • To evaluate the association between pediatric hypercholesterolemia and family history of premature myocardial infarction (MI).

Main Methods:

  • Routine screening identified 500 children with total cholesterol > 5.2 mmol/L (200 mg/dL).
  • Lipoprotein profiles were analyzed to confirm and characterize lipid abnormalities.
  • Family history of premature MI (before age 55) was recorded.

Main Results:

  • 85% of children with elevated total cholesterol had a definable lipid disorder.
  • Common phenotypes included type IIa (292 children), type IIb (99), and type IV (25).
  • Low HDL-C (< 0.9 mmol/L) was found in 20 children; only 5% met criteria for hypercholesterolemia based on high HDL-C (> 1.8 mmol/L).
  • 32% of children had a family history of premature MI.

Conclusions:

  • Pediatric hypercholesterolemia is frequently associated with underlying lipid disorders.
  • Family history of premature MI is common in children with elevated cholesterol.
  • Universal cholesterol screening after age 3 and lipoprotein profiling for elevated levels are recommended.
