Summary

Corpora amylacea (CA) can rarely present as a distinct brain lesion on MRI. This case highlights the importance of considering CA in the differential diagnosis of unexplained focal brain abnormalities.

Background:

  • Corpora amylacea (CA) are glycogen-rich bodies typically found in astrocytic processes within the central nervous system.
  • While CA accumulation is associated with aging and certain neurological diseases, massive localized collections are uncommon.

Observation:

  • A 49-year-old woman with migraines presented with a left parahippocampal lesion on MRI, initially suspected to be a low-grade glioma.
  • Surgical resection revealed a focal, high-density accumulation of CA, with no evidence of neoplasm, ischemia, or hypoxic injury.

Findings:

  • This case demonstrates that localized, high-density corpora amylacea can mimic intrinsic brain lesions on MRI.
  • The imaging characteristics of this CA accumulation were suggestive of a space-occupying lesion.

Implications:

  • Corpora amylacea should be considered in the differential diagnosis for patients with MRI findings of nonenhancing, space-occupying brain lesions.
  • This case expands the known spectrum of clinical presentations associated with corpora amylacea.
