Giant paravertebral myxoma.

Summary

This case report details a rare, giant intramuscular myxoma (IMM) in the paravertebral muscles, a benign soft tissue tumor. The massive tumor was successfully excised, leading to a full recovery for the patient.

Background:

Intramuscular myxomas (IMMs) are rare benign soft tissue tumors of mesenchymal origin.
While common in muscles, they are exceptionally rare in the paravertebral region and typically measure less than 5 cm.

Observation:

A 70-year-old woman presented with back pain, waist asymmetry, and a large right paravertebral mass.
Initial CT-guided biopsy led to a misdiagnosis of juxtafacet synovial cyst.
The mass measured approximately 15 cm, significantly larger than typical paravertebral myxomas.

Findings:

Histological examination confirmed the mass as a low-grade myxoid neoplasm with features of an intramuscular myxoma.
This represents the fifth reported case of a paravertebral IMM and potentially the largest documented.
