Summary

Computed tomography (CT) shows potential for diagnosing deep-infiltrating endometriosis. The tethered appearance of the rectum was the most specific CT finding for this condition.

Background:

  • Deep-infiltrating endometriosis (DIE) is a debilitating gynecological condition.
  • Accurate preoperative diagnosis of DIE is crucial for effective management.
  • Current imaging modalities have limitations in fully depicting DIE.

Purpose of the Study:

  • To evaluate the feasibility of computed tomography (CT) in identifying deep-infiltrating endometriosis.
  • To assess the diagnostic performance of specific CT findings suggestive of DIE.

Main Methods:

  • Retrospective analysis of CT scans from 54 patients with confirmed endometriosis.
  • Evaluation of CT findings including tethered rectum, fat stranding, uterosacral ligament thickening, and uterine position.
  • Independent and blinded review by two radiologists.

Main Results:

  • Deep-infiltrating endometriosis was pathologically confirmed in 63% of patients.
  • Tethered appearance of the rectum demonstrated the highest specificity (90.0%) for DIE.
