Summary

Anomalous left coronary artery arising from the pulmonary artery (ALCAPA) is a rare congenital heart defect. In this case, high pulmonary artery pressures preserved left ventricular function despite severe mitral regurgitation.

  • High pulmonary artery pressures may play a protective role in ALCAPA by preventing myocardial ischemia.
  • Understanding these mechanisms is crucial for diagnosing and managing ALCAPA in older children.
