Summary

Background:

Dissociated vertical deviation (DVD) is clinically recognized, but its horizontal and torsional components are often overlooked.
Dissociated horizontal deviation (DHD) is a proposed label when the horizontal misalignment is prominent.

Observation:

This study details six patients with significant DHD requiring medical attention.
Surgical interventions included unilateral or bilateral lateral rectus recession, with or without superior rectus recession.

Findings:

Surgical treatment, primarily lateral rectus recession, proved effective for DHD.
The darkening wedge test successfully demonstrated the Bielschowsky phenomenon in the horizontal plane for DHD.

Implications:

This research expands the understanding of DHD, emphasizing its horizontal and torsional aspects.
The darkening wedge test offers a novel diagnostic method for identifying DHD and the Bielschowsky phenomenon.
Effective surgical strategies can significantly improve outcomes for patients with DHD.
