Background:

  • Spirometry showed normal major values but an abnormal flow-volume curve.
  • Bronchial provocation tests were negative, and initial treatments failed.

  • Chest X-rays revealed mediastinal widening and tracheal displacement.
  • Nuclear magnetic resonance imaging identified a right aortic arch.
  • The right aortic arch was causing tracheal strangulation.

Implications:

  • A right aortic arch can present with respiratory symptoms due to tracheal compression.
  • Unexplained airway obstruction in adults may warrant advanced imaging.
  • This case highlights the importance of considering vascular anomalies in respiratory diagnostics.
