Background:

Donor-specific anti-HLA antibodies (DSA) in renal transplant recipients are linked to antibody-mediated rejection and reduced graft survival.
Targeting plasma cells with bortezomib is a potential desensitization strategy.

Observation:

This study assessed the efficacy of a single cycle of bortezomib in four kidney transplant recipients with sub-acute antibody-mediated rejection and high DSA levels.
Bortezomib was administered at 1.3 mg/m2 for four doses as the sole desensitization therapy.

Findings:

Bortezomib treatment did not lead to a significant reduction in DSA Mean Fluorescence Intensity (MFI) within 270 days post-treatment.
The proteasome inhibitor failed to decrease DSA levels in sensitized kidney transplant recipients.

Implications:

A single cycle of bortezomib monotherapy may not be sufficient to reduce DSA levels in highly sensitized kidney transplant recipients.
Further research is needed to explore combination therapies or alternative dosing strategies for bortezomib in this context.
