Temporomandibular joint dehiscence can cause acute middle ear effusions by herniating joint contents into the Eustachian tube. This previously unreported cause highlights the importance of considering the temporomandibular joint in diagnosing ear effusions.

Area of Science:

  • Otolaryngology
  • Radiology
  • Anatomy

Background:

  • Acute middle ear effusions typically direct attention to the nasopharynx and skull base.
  • Understanding the anatomical relationships between the temporomandibular joint and the middle ear is crucial for diagnosis.

Observation:

  • Two patients presented with acute middle ear and mastoid effusions.
  • In both cases, dehiscence of the temporomandibular joint was observed.

Findings:

  • Herniation of temporomandibular joint contents occurred posteromedially through the dehiscence.
  • The herniated material obstructed the protympanum, the entrance to the Eustachian tube.

Implications:

  • Temporomandibular joint dehiscence is a novel cause of acute middle ear and mastoid effusions.

Related Experiment Videos

  • This finding expands the differential diagnosis for acute otitis media and mastoiditis.
  • Radiological and clinical evaluation should consider the temporomandibular joint in cases of unexplained middle ear effusions.
