Progressive ventricular expansion in chronic poor-outcome schizophrenia

In chronic schizophrenia, lateral ventricular enlargement is common. Progressive posterior horn enlargement, however, is more pronounced in patients with poor outcomes, indicating a link to functional decline.

- Schizophrenia patients showed significantly enlarged lateral ventricles compared to healthy controls at baseline and follow-up.
- No significant differences in overall longitudinal ventricular expansion were found between schizophrenia patients and controls.
- The poor-outcome (Kraepelinian) schizophrenia group exhibited more pronounced progressive enlargement of the posterior horn than the good-outcome group.
Conclusions:
- Excessive ventricular enlargement in chronic schizophrenia may be specifically associated with poor functional outcomes.
- Posterior horn enlargement progression could serve as a potential biomarker for poor functional prognosis in schizophrenia.
