Summary

Recurrent herpes simplex virus type 2 (HSV-2) meningitis can occur in older adults, not just young individuals. This condition should be considered in the differential diagnosis of aseptic meningitis in the elderly population.

Background:

  • Recurrent meningitis due to herpes simplex virus type 2 (HSV-2) is typically observed in younger, sexually active adults.
  • The clinical presentation and demographic distribution of recurrent HSV-2 meningitis require further investigation.

Observation:

  • A case report details a 78-year-old patient experiencing recurrent HSV-2 meningitis 57 years after their initial genital herpes simplex infection.
  • A literature review of 223 patients with recurrent HSV-2 meningitis revealed that 5% were over 60 years old and 19% were over 50 years old.

Findings:

  • The study highlights that a significant proportion of recurrent HSV-2 meningitis cases occur in older individuals.
  • Age is not a limiting factor for the development of recurrent HSV-2 meningitis.

Implications:

  • Herpes simplex virus type 2 (HSV-2) meningitis should be included in the differential diagnosis for aseptic meningitis in elderly patients.
  • Clinicians should maintain a high index of suspicion for HSV-2 meningitis in older adults presenting with aseptic meningitis symptoms.
