Background:

Malignant glaucoma is a rare but severe form of glaucoma.
Understanding the contralateral eye's status is vital for patient management.

Observation:

Fellow eyes of 9 patients with unilateral malignant glaucoma were examined.
The fellow eyes showed varied conditions post-diagnosis.

Findings:

3 fellow eyes were normal.
3 fellow eyes showed intermittent primary angle closure glaucoma.
3 fellow eyes presented with absolute glaucoma.

Implications:

Fellow eye presentation impacts treatment strategies for malignant glaucoma.
Early detection of fellow eye changes can prevent vision loss.
Further research into the pathogenesis of fellow eye involvement is warranted.
