Summary

Transthoracic echocardiography (TTE) can detect hemodynamic changes during spontaneous breathing trials (SBT). Echocardiographic indices like depressed left ventricular ejection fraction (LVEF) and shortened deceleration time of E wave (DTE) can predict cardiac-related weaning failure.

Background:

Purpose of the Study:

  • To assess transthoracic echocardiography's (TTE) ability to detect hemodynamic effects of SBT.
  • To identify echocardiographic indices predictive of cardiac-related weaning failure.

Main Methods:

  • Transthoracic echocardiography (TTE) performed pre- and post-SBT in 117 patients.
  • Measured mitral E/A velocities, E wave deceleration time (DTE), E' wave velocity, and left ventricular (LV) stroke volume.
  • Compared TTE parameters at baseline and during SBT, stratified by LV ejection fraction (LVEF) and weaning outcomes.

Main Results:

  • SBT significantly increased cardiac output and E/A, and shortened DTE.
  • Patients with LVEF <35% had shorter DTE and higher E/E' at baseline.
  • Cardiac-related weaning failure (87%) was associated with lower baseline LVEF and higher E/E'.

Conclusions:

  • TTE effectively detects SBT-induced hemodynamic changes.
  • Pre-SBT TTE identifying depressed LVEF, shortened DTE, and increased E/E' can predict cardiac-related weaning failure.
  • Further research is needed to evaluate the clinical impact of this TTE-based screening strategy.
