Summary

Distally pedicled osteocutaneous fibular flaps enable successful limb salvage for calcaneal malignancy. This technique provides good tumor control and functional restoration, with minimal complications in most patients.

Minimally Invasive Isolated Limb Perfusion (MI-ILP) for Locally Advanced Melanomas and Sarcomas of the Extremity

  • Functional outcomes assessed using Musculoskeletal Tumor Society (MSTS) and American Orthopaedic Foot and Ankle Society (AOFAS) scores; mean follow-up was 50.4 months.
  • Main Results:

    • All patients achieved wide resection margins; no local recurrence was observed.
    • Four patients had no evidence of disease; one had lung metastasis but was alive with disease.
    • High functional scores (average MSTS 93: 83.2%, AOFAS: 86.4%) with minimal limp in four patients.
    • Fibular flaps survived in all cases with successful bone union (mean 6.2 months) and full weight-bearing (mean 8.6 months).
    • Two patients experienced minor surgery-related complications (hematoma, skin margin necrosis).

    Conclusions:

    • Distally pedicled osteocutaneous fibular flap reconstruction is a successful limb salvage strategy for calcaneal malignancy.
    • The procedure offers satisfactory local tumor control and effective functional restoration of the lower extremity.
    • This technique demonstrates good oncologic and functional outcomes, making it a viable option for complex calcaneal tumor resections.
