Summary

Confluent soft drusen in the macula are linked to reduced retinal sensitivity. Morphologic changes, including retinal pigment epithelium (RPE) and photoreceptor inner/outer segment junction irregularities, correlate with vision loss.

Background:

  • Confluent soft drusen are a common finding in age-related macular degeneration.
  • Understanding their impact on retinal function is crucial for predicting disease progression.

Purpose of the Study:

  • To investigate the relationship between retinal sensitivity and structural changes in the macula of eyes with confluent soft drusen.

Main Methods:

  • Retrospective review of 21 patients (22 eyes) with confluent soft drusen.
  • Spectral-domain optical coherence tomography (SD-OCT) for macular imaging.
  • Microperimetry to assess retinal sensitivity.

Main Results:

  • SD-OCT revealed retinal pigment epithelium (RPE) protrusion corresponding to drusen.
  • Irregularities in the photoreceptor inner/outer segment (IS/OS) junction were observed in 77.3% of eyes.
  • Reduced retinal sensitivity was significantly associated with RPE and IS/OS irregularities.

Conclusions:

  • Confluent soft drusen are associated with focal areas of decreased retinal function.
  • Irregularities in the RPE and IS/OS junction are key indicators of reduced retinal sensitivity in these eyes.
