Summary

Splenosis, the spread of spleen tissue after injury, can occur in the ovary. This asymptomatic condition may mimic other diseases but can retain spleen function and should not be removed.

Background:

Splenosis, autotransplantation of splenic tissue post-splenic rupture, is increasingly recognized.
It can present asymptomatically and mimic other conditions, notably endometriosis in females.
Ovarian involvement, though rare, is a documented manifestation.

Observation:

A case of asymptomatic ovarian splenosis involving the left ovary is presented.
The ovarian splenosis was an incidental finding accompanying multiple pelvic and serosal splenotic nodules.
This represents the fourth reported case of ovarian splenosis globally.

Findings:

Splenosis can involve the ovary, presenting as an incidental finding.
The condition can occur with widespread pelvic and serosal splenotic implants.
Asymptomatic splenosis may retain splenic function, a crucial consideration in patients with prior splenectomy.

Implications:

Ovarian splenosis diagnosis requires awareness, especially in patients with a history of splenectomy.
Routine excision of splenosis is discouraged due to its potential functional role.
Understanding splenosis presentation is vital for accurate diagnosis and management, avoiding unnecessary interventions.
