Capsule endoscopy in small bowel tumors: a multicenter Korean study

Background:

• Capsule endoscopy (CE) is effective for small bowel lesions but its impact on small bowel tumors, especially in Asian populations, is understudied.
• Limited research exists on the practical diagnostic and therapeutic implications of CE for small bowel tumors.

Purpose of the Study:

• To evaluate the diagnostic and therapeutic impact of capsule endoscopy (CE) in managing small bowel tumors.
• To assess the effectiveness of CE in identifying small bowel tumors compared to conventional radiological studies.

Main Methods:

• A retrospective review of CE records from 14 Korean centers (October 2001–April 2008).
• Analysis of clinical information and CE video images for small bowel tumors in 1332 patients.
• Evaluation of diagnostic yield and therapeutic changes resulting from CE findings.

Main Results:

• Small bowel tumors were diagnosed in 57 (4.3%) of 1332 patients via CE.
• Malignant tumors (33 cases) included adenocarcinomas, lymphomas, gastrointestinal stromal tumors, and metastatic cancers.
• CE exclusively identified 30 tumors (52.6% diagnostic impact), often smaller than those found by radiology; 7 patients (12.3% therapeutic impact) underwent surgery.

Conclusions:

• Capsule endoscopy (CE) offers significant diagnostic advantages for small bowel tumors, revealing lesions missed by conventional radiology.
• CE plays a critical role in altering therapeutic management for patients diagnosed with small bowel tumors.
