Summary

Laparoscopic colorectal surgery did not decrease incisional hernia rates compared to open surgery. However, hernias in laparoscopic cases were smaller and often suitable for laparoscopic repair.

Background:

  • Incisional hernias are a common complication of open abdominal surgery, occurring in 4-18% of cases.
  • These hernias often develop within two years and pose a significant challenge for surgeons.

Purpose of the Study:

  • To prospectively evaluate incisional hernia rates in patients undergoing laparoscopic colorectal resection.
  • To compare hernia rates between laparoscopic and open procedures.

Main Methods:

  • A prospective review of 167 patients undergoing laparoscopic colorectal resection.
  • Standardized wound closure techniques were used for all incisions.
  • Patients were followed up and examined by an independent senior surgeon.

Main Results:

  • Seven percent (12/167) of patients developed an incisional hernia.
  • Most hernias occurred at the specimen extraction site.
  • Ten of the twelve hernias were successfully repaired laparoscopically.

Conclusions:

  • Laparoscopic colorectal surgery does not appear to reduce incisional hernia rates.
  • Hernias resulting from laparoscopic procedures may be smaller and more amenable to laparoscopic repair.
  • Further research is needed to fully understand the long-term implications.
