Area of Science:

  • Cardiovascular Surgery
  • Pediatric Cardiology
  • Congenital Heart Disease

Background:

  • The no-patch repair was the initial surgical approach for complete atrioventricular canal (CAVC).
  • Patch repairs are now more prevalent for CAVC correction.
  • Evaluating alternative techniques to optimize surgical outcomes is crucial.

Purpose of the Study:

  • To assess the safety and efficacy of a no-patch surgical technique for CAVC repair.
  • To compare the outcomes of the no-patch technique with traditional patch repair methods.

Main Methods:

  • A consecutive series of 8 patients with CAVC underwent surgical repair using a no-patch technique.
  • Sutures were placed on the right of the ventricular septal defect crest, through the bridging leaflet, and to the ostium primum defect.

  • Intraoperative and early postoperative outcomes were meticulously documented.
  • Main Results:

    • No early deaths were recorded in the study cohort.
    • All patients maintained sinus rhythm post-surgery.
    • No instances of left ventricular outflow tract obstruction were observed.
    • The no-patch technique demonstrated results comparable to modified single-patch repairs.

    Conclusions:

    • The suture-only no-patch technique is a safe and effective alternative for CAVC repair.
    • This approach offers comparable outcomes to patch repair while potentially reducing surgical time.
    • Further research with larger cohorts may validate these findings and support wider adoption.
