Endoscopic Procedures V: ERCP

Endoscopic Retrograde Cholangiopancreatography (ERCP) is a diagnostic procedure that combines endoscopy and fluoroscopy to diagnose and treat conditions related to the bile ducts, pancreatic ducts, and gallbladder. This procedure is beneficial for identifying and addressing blockages, gallstones, strictures, and tumors within the biliary or pancreatic systems. ERCP is both diagnostic and therapeutic, offering the ability to visualize and treat identified problems in one session.

Area of Science:

  • Interventional Gastroenterology
  • Biliary Interventions
  • Oncologic Palliation

Background:

  • Malignant hilar stenosis poses challenges for stent insertion due to cholangitis risk from contrast agents.
  • Effective palliation requires safe and precise stent placement in the obstructed biliary system.

Purpose of the Study:

  • To assess the efficacy and safety of using air as a contrast medium for unilateral metallic stenting.
  • To evaluate outcomes in patients with type II and III malignant hilar biliary obstruction.

Main Methods:

  • A prospective, single-center pilot study involving 17 patients with malignant hilar obstruction.
  • Unilateral metallic stent insertion using air cholangiography for type II and III obstructions.
  • Weekly patient evaluation for one month post-procedure.

Related Experiment Videos

Main Results:

  • 100% successful stent implantation and biliary drainage was achieved in all 17 patients.
  • No instances of cholangitis were observed.
  • Zero procedure-related mortality and zero 30-day mortality.

Conclusions:

  • Air cholangiography is a safe and effective tool for guiding unilateral metallic stenting in malignant hilar biliary obstruction.
  • This method offers a promising alternative to traditional contrast agents, reducing complication risks.
