Clonazepam use in a patient with end-stage renal disease led to pancytopenia, a severe drop in blood cell counts. Discontinuation of clonazepam resulted in recovery, highlighting the need for blood count monitoring in patients on this medication.

Background:

  • A 48-year-old female with end-stage renal disease and chronic anemia on hemodialysis was prescribed clonazepam for myoclonus.
  • Two weeks after initiating clonazepam, she was hospitalized with neutropenic fever, thrombocytopenia, and worsening anemia.

Observation:

  • Bone marrow examination revealed markedly hypocellular marrow (10-20% cellularity).
  • The patient's symptoms of thrombocytopenia and neutropenia gradually improved within 1-2 weeks after discontinuing clonazepam.

Findings:

  • This case represents the first reported instance of pancytopenia associated with clonazepam use.
  • The findings suggest a potential causal link between clonazepam and the development of pancytopenia in susceptible individuals.

Implications:

  • Patients prescribed clonazepam, especially those with pre-existing hematological conditions or on hemodialysis, should undergo regular complete blood count monitoring.
  • Early detection and discontinuation of clonazepam may prevent severe hematological complications like pancytopenia and thrombocytopenia.
