A hair follicle or HF is a small part of the skin that produces the hair shaft. Paul Gerson Unna was the first to observe a bulge in the human hair follicle's outer root sheath (ORS). The papillary and reticular dermis are the two layers of the dermis. They are made of connective tissue with fibers of collagen extending from one to the other, making the border between the two somewhat indistinct. Background:

Hibernomas are rare benign tumors originating from brown adipose tissue.
Lipoma-like hibernoma (LLH) is a rare histologic variant with limited documented characteristics.
Information on LLH clinicopathologic features and treatment is scarce.

Observation:

A 56-year-old male presented with a painful pelvic/buttock lipoma-like hibernoma.
The LLH recurred locally nine years after initial marginal excision.
The recurrence was managed with partial excision and embolization, providing symptomatic relief.

Findings:

This report details the first documented case of recurrent lipoma-like hibernoma.
Lipoma-like hibernoma predominantly affects individuals in their fifth and sixth decades, with a slight male predilection.
Magnetic resonance imaging findings for LLH are variable, mirroring fat signals with potential septations and contrast enhancement.

Implications:

This case underscores the potential for local recurrence of lipoma-like hibernoma.
Further research is needed to fully understand the behavior and optimal management of lipoma-like hibernoma.
Long-term follow-up may be necessary for patients treated for lipoma-like hibernoma.
